After-School Science Enrichment Club
Late Pick-up Policy and Acknowledgement Form
The Mad Science After-School Enrichment Club is a 60-minute program. Class ends at a specific time, as noted on your registration flyer. Our instructors are not child caregivers, and are not compensated for staying after to watch your child when you do not arrive by the pickup time indicated for your class.
You hereby acknowledge that:
- A late fee of $1.00 per minute per child will be assessed for a late pick up. Fees will be charged to the credit card used for registration. If no card number is available, an invoice will be sent in the mail. The invoice must be paid in full before the next class, or the child will not be allowed to attend until the balance is paid.
- If a parent or authorized person does not arrive within 15 minutes after class ending, the Mad Science instructor will assume an emergency exists and will begin to call emergency contacts for your child.
- If no designated person can be reached, or no person contacted arrives within a reasonable time, the local Police Department will be called to pick up the child.
- If your child is picked up late more than twice in single five-week session, the child will be dropped from the program and no refund will be given.
It is very important to give us updated contact information for your child. Any child who is not picked up will be under the supervision of the Mad Science program instructor until the parent, emergency contact or the authorities arrive.
